Dry Needling After Surgery: A Valuable Tool for Recovery

When people think about surgery, they often think the procedure itself is what “fixes” the problem—and structurally, that’s true.
But surgery only addresses the damaged tissue.
It does not automatically resolve everything happening around it.
At Undefeated Physical Therapy and Performance, we see this often with throwing athletes recovering from Ulnar Collateral Ligament Injury (UCL) repairs or Tommy John surgery. The ligament may be repaired, reconstructed, or reinforced, but the muscles surrounding the elbow, shoulder, forearm, and even the wrist are often still tight, guarded, painful, and protective.
That’s where Dry Needling can be a powerful tool.
Dry needling helps address the muscular side of recovery—something surgery alone cannot do.
Surgery Fixes Structure. Dry Needling Helps Restore Function.
After surgery, the body naturally goes into a protective state.
This protective response often leads to:
Increased muscle guarding
Trigger points
Pain
Stiffness
Reduced mobility
Altered movement patterns
Even when the repaired tissue is healing normally, these muscular restrictions can slow down progress.
Dry needling allows us to target those dysfunctional muscles directly to help reduce tension, improve blood flow, and restore movement.
Dry Needling Can Help Reduce Post-Operative Pain
Pain management is one of the biggest challenges after surgery.
While pain medication can be necessary, reducing the amount someone needs is always a positive.
Dry needling can help decrease pain by reducing muscular tension and calming down irritated tissues.
A 2024 randomized controlled trial on patients following rotator cuff repair found that adding dry needling to rehabilitation improved pain and functional outcomes compared to sham needling—especially during the early phases of recovery.
That matters.
Because the sooner we can reduce pain, the sooner we can improve movement.
Improving Range of Motion After Surgery
One of the most common limitations after surgery is stiffness.
And many times, that stiffness is not coming from the joint itself—it’s coming from the muscles.
Hypertonic muscles, trigger points, and protective guarding can limit motion even when healing is progressing normally.
Research shows dry needling may help:
Improve range of motion
Decrease muscular tension
Improve tissue extensibility
Reduce guarding
This is especially important for overhead athletes where restoring full motion is critical.
For example:
After elbow surgery, we may dry needle the wrist flexors or extensors to improve tendon mobility.
We may also needle the shoulder or pec muscles to help improve shoulder external rotation, flexion, and overall movement quality.
The goal is simple:
Treat the chain, not just the site.
Blood Flow Matters
If you’ve read any of my blogs before, you know I’m always talking about blood flow.
And for good reason.
Blood flow is what delivers the nutrients needed for healing:
Oxygen
Sodium
Potassium
Magnesium
White blood cells
Growth factors
Dry needling creates a local response that increases circulation to the area.
Better blood flow means better healing.
It helps clear out metabolic waste, improve tissue quality, and support recovery at a higher level.
Restoring the Neuromuscular Connection
Recovery isn’t just about healing tissue.
It’s about getting the brain and muscles communicating effectively again.
After surgery, pain and swelling can disrupt that connection.
Dry needling can help improve neuromuscular function by improving local circulation and reducing inhibitory muscle guarding.
This helps:
Improve muscle activation
Improve contraction quality
Restore movement patterns
Build strength more efficiently
This is especially important when returning athletes back to throwing, hitting, and high-level performance.
When We Need to Be Cautious
Like any tool, dry needling has to be used at the right time.
Just because it can help doesn’t mean we force it too early.
The biggest thing we want to do is reduce infection risk.
Even though dry needling is minimally invasive, it still punctures the skin.
In the early post-operative phase, the surgical site is vulnerable.
That’s why we wait until:
The incision is fully healed
There are no signs of infection
The tissue is ready to tolerate treatment
Protecting the surgical site always comes before chasing short-term symptom relief.
Later in the recovery process, dry needling can even be used directly over or around scar tissue to improve scar mobility, tissue extensibility, and the quality of healing in the layers underneath the skin.
We are also cautious around:
Surgical hardware
Altered anatomy
Areas with excessive swelling
High-irritability tissue
Clinical reasoning always matters.
Final Thoughts
Dry needling is not the thing that heals the surgery.
But it can be one of the best tools to improve the environment around it.
When used appropriately, it can help reduce pain, improve motion, restore muscle function, improve blood flow, and accelerate the return to movement.
